Partners In Care Veteran Support Services
Veterans hold a very special place in the hearts of the team at Partners In Care. Veterans have devoted themselves in service to our country, and we are honored to serve them with the best possible care.
Our Veterans Outreach program is unique among hospice organizations in the region. We are one of only three hospice organizations in the state of Oregon to be recognized by the National Alliance for Home Care as the highest level of ‘We Honor Veteran’ programs due to the exceptional and tailored care provided to veterans on our services and in the community. Our team of support specialists is committed to providing specialized care for each of our military member patients and their families. We understand the unique health care and mental health needs and challenges of military veterans because some of our staff and our veteran volunteers served in the armed forces and they continue to serve and find ways to provide assistance to the veteran population in our hospice, home health, palliative care, and transitions programs.
Special Veteran Services
In addition to the many Partners in Care medical and support services we provide to all patients, we work with veterans and their families to ensure they receive the benefits they qualify for regarding their military service. Our Veterans Outreach Coordinator works directly with the Department of Veterans Affairs, both state and local, to find answers to your questions, help navigate serious illness, and ensure you or your veteran are receiving the best possible care. We also connect families with other veteran programs that can enrich their bond, memories, and relationship with their loved one.
We Honor Veterans Program
Partners in Care is a nationally certified, five-star member of the National Hospice and Palliative Care Organization We Honor Veterans Program. We are dedicated to providing the highest level of Veteran-centric services that address their needs through our Vet-to-Volunteer program, recognition ceremonies, and help navigating VA benefits.
